INSTRUCTIONS THAT NEED TO BE FOLLOWED BY MARINE CANDIDATES WHEN COMING IN FOR TEST FOR SEAFARER MEDICAL EXAMINATION AT SHIPCREW MEDICAL CENTRE, KARNATAKA-BENGALURU
- SEVEN latest passport sized colour photographs will be required.
- We will need CDC and PASSORT both documents FIRST AND LAST pages of xerox copy and bring your originals to establish your identity.
- If the candidate who does not have CDC and PASSPORT to show their identity. Such candidate have to bring Aadhaar Card both side xerox copy with originals.
- If available bring your previous Medical fitness certificate along with complete record of history.
- In order to have accurate test result it is recommendent to come to the centre without having breakfast.
- We will need you to fill in the Self Declaration form about your present health,any treatment ,illness without concealing any facts in our centre.
