INSTRUCTIONS THAT NEED TO BE FOLLOWED BY MARINE CANDIDATES WHEN COMING IN FOR TEST FOR SEAFARER MEDICAL EXAMINATION AT SHIPCREW MEDICAL CENTRE, KARNATAKA-BENGALURU

SEVEN latest passport sized colour photographs will be required.
We will need CDC and PASSORT both documents FIRST AND LAST pages of xerox copy and bring your originals to establish your identity.

*** NOTE ***

If the candidate who does not have CDC and PASSPORT to show their identity. Such candidate have to bring Aadhaar Card both side xerox copy with originals.

If available bring your previous Medical fitness certificate along with complete record of history.
In order to have accurate test result it is recommendent to come to the centre without having breakfast.
We will need you to fill in the Self Declaration form about your present health,any treatment ,illness without concealing any facts in our centre.
